Hair Extension Service Agreement

This Hair Extension Service Agreement ("Agreement") is made effective as of 03/13/2025, by and between Hope Newnam | Tulsa Hair Extension Pro ("Stylist") of 5800 e Skelly Dr. STE 170 , Tulsa OK 74135 and the client specified at the bottom of this agreement ("Client").

1. DESCRIPTION OF SERVICES: The Stylist agrees to provide hair extension services ("Services") as agreed upon and described during the consultation session.

2. MODEL RELEASE: The Client grants the Stylist the right to photograph, record, or reproduce images of the Client's hair and appearance before, during, and after the Services. These images may be used for the Stylist's portfolio, marketing, and promotional purposes without compensation to the Client.

3. RISKS ACKNOWLEDGMENT: The Client acknowledges that there are risks associated with hair extension services, including but not limited to hair breakage, hair loss, scalp irritation, and allergic reaction. The Client agrees that the Stylist is not liable for any such adverse reactions or outcomes.

4. MAINTENANCE AND CARE: The Client agrees to follow the Stylist's recommended maintenance and care routine to ensure the longevity and quality of the hair extensions. Failure to follow these guidelines may result in diminished product quality and longevity. The Stylist is not responsible for any damage resulting from the Client's failure to adhere to these recommendations.

5. NATURAL HAIR DAMAGE: The Client acknowledges that while the Stylist will perform the service with utmost care, the process of applying and maintaining hair extensions can potentially lead to natural hair damage. The Client agrees not to hold the Stylist responsible for any natural hair damage or hair loss that might occur during or after the service.

6. CHOICE OF PRODUCTS: The Client agrees that the Stylist has the sole discretion to determine which products, including but not limited to hair extensions, bonding materials, and maintenance supplies, will be used. These products have been selected based on quality and suitability for the Client's hair.

7. REFUND POLICY: All services rendered by the Stylist are provided on a non-refundable basis. In the event of any dissatisfaction with the service, the Client agrees to notify the Stylist within 48 hours to discuss potential adjustments. However, no refunds will be provided.

8. DEPOSIT: The Client agrees to pay a deposit in the amount of [Amount] to secure the appointment for the Services. This deposit is non-refundable under all circumstances, including but not limited to instances where the Client decides to cancel the appointment, fails to attend the appointment, or requests to reschedule the appointment. The Client acknowledges and agrees that no refunds will be issued for the deposit for any reason.

9. TERMINATION: Either party may terminate this Agreement at any time by providing written notice to the other party. If the Client terminates the Agreement before the service is completed, the Client acknowledges that they are ineligible for any kind of refund, including deposits.

10. ENTIRE AGREEMENT: This Agreement contains the entire understanding and agreement between the Client and the Stylist and supersedes any and all prior or contemporaneous negotiations, discussions, agreements, understandings, and representations, whether oral or written, between them.

11: NON-DISPARAGEMENT CLAUSE: The Client agrees to refrain from public statements, either online or offline, that negatively impact the Stylist’s professional reputation, practice, or business operations.

12. SERVICE RATE: The Client agrees to pay the Stylist as per the agreed rate for the hair extension services.

13. GOVERNING LAW: This Agreement shall be governed by the laws of OK.

14. AMENDMENT: Any amendment to this Agreement must be in writing and signed by both parties.

15. SIGNATURES: By signing below, both parties acknowledge that they have read, understood, and agree to the terms and conditions set forth in this Agreement.
